Combination Therapy Vs. Facelift Surgery

Question: Another plastic surgeon advised laser, fillers and Botox for my facial aging. My facelift from 10 years ago was a single procedure. While I don’t want a second facelift, would I be better off with one instead of doing multiple other procedures?

Answer: A common misconception is that facelifts can achieve the same results as fillers, lasers and Botox. While one can delay the need for a facelift and, in some cases, avoid one altogether if less-invasive rejuvenation treatments are begun early, facelifts are neither forever nor are they an adequate substitute for many other procedures.

Botox can soften upper facial wrinkles caused by overactive muscles. Facelifts won’t treat such wrinkles. Similarly, injectable fillers can soften lines and restore facial volume loss, none of which can be accomplished with a facelift.

There is a key difference between loose skin and poor skin elasticity. Ultherapy and Infini RF work, not by directly lifting tissues as with facelift surgery, but by increasing collagen and elastin to intrinsically tighten the skin. In fact, if a patient with poor skin elasticity has a facelift, the results may not only look pulled but also be short-lived. On the other hand, with significant loose skin, a facelift can be more effective than skin tightening devices.

Even with facelift surgery, many patients additionally need some combination of fillers, Botox or laser. Single- treatment approaches work best in the early age groups of the 30’s and 40’s. Mature patients typically require combination therapy with combo-procedures costing more than single-modality procedures.

The good news is that longer-lasting, more effective minimal-invasive procedures do exist. They tend to come with higher price tags due to their advanced technology.
