The 3 Types Of Facial Sagging

Posted: Sunday, Jan. 1, 2017

By Anita Mandal, M.D.
Question: I have sagging jowls. I’ve read that putting Bellafill in my hollow cheeks can lift my droopy jowls? How do I know if I should get a facelift or fillers to help my sagging?

Answer: Facial sagging can be divided into 3 categories. Which category you fall into depends on how you age and can also determine your choice of treatment.

There are several different causes of sagging jowls. One such cause is the loss of fat and eventually bone from our upper face i.e. the cheeks and temples. This volume loss results in loss of support of the overlying skin which then begins to sag. Restoration of facial volume with injectable fillers includes Perlane, Juvederm, Radiesse and Sculptra, all of which are temporary and require costly maintenance. Bellafill, the 5-year filler, while more costly upfront, tends to be more cost-effective in the long term, giving amongst the most natural results.

A second cause of facial sagging is poor skin elasticity due to loss of collagen and elastin within the skin itself. Sagging from loss of skin elasticity can be improved with non-surgical skin tightening devices such as Ultherapy and Infini, both of which work by stimulating collagen and elastin. Infini tends to be more effective than Ulthera. Such devices work best when loss of tissue laxity is mostly limited to the skin layers and is mild to moderate such that it does not require surgical excision of excess skin.

The third type of facial sagging comes from greater loss of skin elasticity which can be associated with sagging of the deeper tissues such as the muscles. Sagging of the deeper tissues can further pull on the overlying skin. With moderate to severe sagging, surgical procedures are typically needed. Such procedures range from a full facelift to the Laser WhisperLift which is less invasive surgery, uses tiny micro-incisions, local anesthesia and averages 4-5 days of downtime.

Everyone ages differently and some patients may have 1, 2 or all 3 types of facial sagging. Whether a single procedure or combination of procedures is most suitable for you is best determined after a thorough consultation with a board certified Facial Plastic Surgeon who understands the intricate details and anatomy of your unique facial aging.
