When A Facelift Can Make You Look Worse?

Question: I had a facelift 10 years ago. I was happy with it, but now I see some sagging in my neck and lower face. I hear that second facelifts can sometimes make you look worse or unnatural? Are options available that can help tighten and lift my face and neck, yet still keep me looking natural?

Answer: It is typically better to have a facelift when the elasticity of your face and neck skin is still good. As you age, the collagen and elastin fibers of the skin weaken and break down with the skin losing its “memory” and ability to stay in a certain position. When this happens, if you have a facelift in which the skin is pulled and repositioned, it is less likely to stay in the same tightened position and more likely to sag soon after surgery.

Another downside of performing a facelift when the tissues have poor elasticity is that the skin can end up looking pulled or with numerous ridges once the swelling from surgery goes away. Facelifts do not improve a poor skin elasticity problem. An effective way to improve skin elasticity is by stimulating production of skin collagen and elastin. Infini RF is a non-surgical device commonly used in my office to help tighten and lift sagging skin. Infini RF works, not by pulling and removing excess skin as with a facelift, but by intrinsically tightening and lifting from within the skin itself. Unlike a facelift, there is no downtime with Infini RF and the risks are much lower.

Another consideration is the loss of facial volume that occurs with facial aging. Volume loss can make the skin sag. Think of the aging face as a gradually deflating balloon which becomes gradually more wrinkled as it loses air. When a facelift is performed on a patient with significant facial volume loss, the end result can be a pulled or unnatural looking face. Treatment in this case should include facial volumization with injectable fillers rather than just facelift surgery.

Gone are the days of just recommending surgery for facial sagging which has several different causes and treatments.
